Our Mission: Life Journey Partners of America is a Non-profit Organization committed to building and supporting  Caregiving Communities within existing communities to address gaps in care and provide education and training necessary to care for individuals who would otherwise be unable to remain in their homes due to physical or mental disabilities, regardless of the ability to pay. 

Our Vision: As our population ages, more people will need caregiving, and many will be unable to pay for the care needed, requiring care by family and friends. We envision a community supportive of caregivers and committed to finding caregiving solutions to cover gaps in care, including training of paid and unpaid caregivers, funding to cover gaps in essential care and a House Share Program, developed to match individuals who will flourish in a shared living arrangement, serving the needs of the home insecure and those at risk while living alone.

Our team combines years of experience in Medicine, Nursing, Elder law, Care Coordination and Advocacy. Our work targets the gaps in care and support that have been identified by patients, family caregivers, and professionals in the field of elder care and of those living with a disability. 


Karen Willfahrt

Director

Karen Willfahrt is a Naturopathic Physician and Registered Nurse. Her years of experience in the home include work as a Hospice Nurse and in Private Duty nursing, which have informed her of the gaps in support to patients and their families. She  supports a Holistic Care model as a solution. 


Dianne McNamara

Elder Law and Advocacy Consultant

Dianne O’Donnell McNamara is an Attorney, Consultant and Advocate. She is dedicated to elders’ person-centered care, well-being and quality of life. Dianne specializes in aging services, public benefits, long-term care, guardianships/conservatorships and estates & trust administration.

Crystal Sanchez-Griffiths

Care Coordination Consultant

Crystal Sanchez-Griffiths is experienced as a Physician and Community Liaison, developing relationships crucial to patient care and aftercare. Crystal has worked in various areas of healthcare and  is passionate about Patient Advocacy and Care Coordination.
